Medication Therapy

We offers customized educational interventions to prevent underutilization, overutilization, inappropriate use, and abuse of drugs.

We have a systematic process of collecting patient-specific information, assessing medication therapies to identify medication-related problems, developing a prioritized list of medication-related problems, and creating a plan to resolve them with the patient, caregiver and /or prescriber.

We are contracted with

This method enhances our communication with patients and makes a difference in their health outcomes: Encourages patients to commit to taking medications according to their physicians' recommendations.
